The Last Chapter: Ins and Outs of Advance Care Planning In-Person

At any time in your life, you may be unable to communicate your health care choices as a result of an injury or serious illness. Advance care planning encourages understanding and defining possible healthcare choices, talking about decisions with loved ones and doctors, and putting plans in writing to be ready if needed. It is a gift you can give to yourself and those you love.

Having a plan will make it easier for you, your doctor and your loved ones to ensure your wishes are honored.

This free session helps identify health care choices and suggests ways to begin communicating them to family and health care providers. 

It explains the use of Advance Directives (Living Will and Health Care Power of Attorney forms) to legally express your choices. Registration is required due to space limitations.
